From what I understand, EAP for grad school is equal to six hours in-state tuition and fees at UM College Park. According to their Web site, that would be $3,163.04 for Fall 09/Spring 10. Is this correct? You are correct. The maximum EAP benefit can exceed the $2,020 for non-bargaining staff by an amount such that total reimbursement would be the greater of $2,020 or the amount equal to the University of Maryland College Park rate for in-state tuition and fees for graduate or undergraduate coursework for each credit, up to a maximum of six (6) credits provided that the total EAP benefits payable to eligible non-bargaining staff for FY10 shall not exceed $312,798. I have heard that I may get an additional 1-1/2 hours of leave each week to participate in wellness activities. How does this work? Staff wishing to request time to participate in wellness activities must match non-duty time with duty time. No more than one-half of the total wellness time may be released time; employees may use as much of their non-duty time as practical. Time for wellness activities must not exceed one hour at a time including time needed to get to and from the activity and time to change clothes. Staff members are encouraged to participate in wellness activities a minimum of three days a week. A maximum of 1-1/2 hours of duty time per week can be approved for these activities. You must complete a "Staff Request for Wellness Activities Form" available on the wellness page that must be approved by your supervisor in order to utilize "release time" for wellness activities: http://www.montgomerycollege.edu/wellness/forms.html. I have the Cigna POS plan. Are eye exams covered for routine vision screening? Yes, the POS (managed care plan) covers vision exams with a $5 co-pay. You are covered when you visit a VSP in-network provider, employees on the Managed Care plan need to make sure that they inform the provider that they have VSP (through the CIGNA POS) so that claims will be submitted correctly. Providers can be found on the VSP Web page at http://www.vsp.com. Cigna sent me an affidavit to sign to document that my dependant is still a full time student. What do I need to send from his college; it is not mentioned. You only need to complete the affidavit with the requested information and mail it back to Cigna. No documentation from the dependant's college is required at this time. It must be returned as soon as possible to avoid termination of coverage.
